Contact Update Form

Please submit this form if you have information such as email, phone number or address that has been updated or has changed. Please submit this form if there is additional information you'd like to provide to Mountain View Chapel.
Updated information should be submitted for each adult in your household, as information for each adult is linked to their individual phone number and email address. 
Important: This should be the same email you use to log in to the MVC app. Updating your email here will also update it in our church database.
If you change your email, it may affect your ability to log in to the MVC app for giving, chatting, and other features.
✅ If your email is correct and up-to-date, you don’t need to change anything.

Please enter your current mailing address.

Even if you don’t typically receive mail from MVC, we ask that you provide your address so we can keep your profile complete.
This helps us with occasional needs like giving statements or sending cards.


Children in Sunday School

If your children regularly attend Sunday School, please list each child’s:
  • Name
  • Grade
  • Graduation Year 
  • Birthdate

Note: If you're unsure about graduation year, that's okay—this form is part of an annual update.
